you with the finest quality health care. We hope that this information will
help you to understand how our office functions as well as your responsibilities.
While in our office we kindly ask that you do not eat, drink or use cellular
phones. While we understand that occasionally it will be necessary to bring
your children with you, we would appreciate if you attempt to make childcare
arrangements.
Insurance With the vast number of insurance
plans, it is the patient's responsibility to understand their insurance
coverage. If you require assistance, we will assist you to the best of our
ability. At your initial visit, you will be asked to present your insurance
card and to complete paperwork regarding your demographic information, etc.
If you do not have an insurance card available, full payment is expected
at the time of service. At regular intervals, you will be asked to update
this information. While we acknowledge that this may be an inconvenience,
it is necessary that we maintain current information.
You are responsible for any deductibles, co-payments and/or any referrals that your insurance company may require. Payment is expected in full at each visit for services not covered by your insurance plan, as well as for all service rendered to patients with insurance plans with which our physicians do not participate.
Obstetrics & Gynecology Associates does not participate with Medicaid. Should you seek our services having been informed of our non participating status, you will be asked to sign a waiver acknowledging that you will personally assume responsibility for payment of all services rendered. Should your insurance change to Medicaid at anytime, it is your responsibility to notify our office immediately.
Co-Payments & Returned Checks Your co-payment is expected at the time of service. All returned checks will be assessed a $25.00 service charge. For your convenience, we accept MasterCard or VISA.
Office Hours Patients are scheduled between the hours of 8:30am and 5:00pm, Monday through Friday, All patients are seen by appointment only. Our office staff answers the telephones between the hours of 9:00am and 5:00pm with the exception of an hour for lunch. The calls will then be handled by our answering service without interruption. One of our physicians is on call 24 hours a day, 7 days a week. Should you have an emergency, our answering service will contact the physician on call. If you believe that you cannot wait for the physician to return your call, you should go to the nearest emergency room.
Ancillary Tests For our patients' convenience, we offer Mammography, Bone Density and Ultrasound testing on-site. We ask that you make necessary childcare arrangements prior to your testing. For safety and liability reasons, we do not permit children in the testing rooms. Children may be left in the waiting room providing an adult accompanies them. Please note that technicians are not authorized to discuss test results.
Laboratory Service Lab specimens collected in our office are sent to an outside laboratory for testing with the exception of pregnancy tests, routine urinalysis and hemocults.
Cancellation Policy If for any reason you must cancel or reschedule your appointment(s), please notify our office as soon as possible (24 hours prior to appointment is required).
Late Arrivals If you arrive late for your appointment(s), we may have to reschedule your appointment(s) or ask you to wait to see if we can accommodate you in our schedule.
Prescription Refills For routine prescription
refills, please leave a message on our prescription refill line. If your
call is received prior to 4:00pm your prescription will be called in the
pharmacy you indicate that business day. Our office policy is not to refill
birth control prescriptions or non-emergent medication during the hours
that our office is closed. We ask that our patients be responsible for monitoring
prescription needs. Please remember that the physician is on-call for emergency
purpose only.
Billing Questions If you have any questions
regarding billing, please call our central billing office at (800) 889-3333
during regular business hours.
Non-patients We cannot answer any clinical
questions or give medical advice to any patient that has not been seen at
this office for the last three years. Patients who have not been seen for
the past 12 months or longer are kindly asked to schedule an annual exam.
